Patient Forms, Map and Directions to Denmark Office

When you arrive at Denmark Dentistry, you will be greeted by our office staff. We will guide you through the registration process. You will be asked to complete Registration and Medical History Forms. (These documents may be downloaded from the Helpful Links section of the website and completed prior to arriving at Denmark Dentistry.) Your information will be entered into our computer system and we will verify your insurance coverage.

Denmark Dentistry maintains electronic records for our visits, so you will be asked to sign an electronic copy of your medical history and an acknowledgement of agreement with our Privacy Act Notice, appointment and payment policies. (These documents may be viewed on our website and are posted in our lobby.) If you have any questions about payment arrangements, insurance, or costs of scheduled treatment, be sure to ask Gilda during your visit.

After you have completed the registration process, one of our Dental Hygienists or Dental Assistants will escort you to one of the practice’s treatment rooms depending on the scheduled treatment. If you are scheduled for routine dental examination and prophylaxis (cleaning), our Dental Hygienists will take the necessary X-rays and perform the cleaning of your teeth; then one of our Dentists will examing your teeth and address any questions or concerns that you may have about your oral hygiene. If you are scheduled for other treatment such as fillings; extractions; root canal treatment; crown and bridge or denture, our Dental Assistants will take the necessary X-rays and then one of our Dentists will answer your questions and discuss your course of treatment with you prior to beginning treatment. If you have any questions or concerns about your dental hygiene or you are not happy with the appearance of your teeth, one of our Dentists will be happy to discuss these issues with you and recommend a course of treatment that will correct any problems or concerns that you have.

Dental X-rays are an important tool in the diagnosis and treatment of dental problems. They do not, however, replace the need for a visual examination of the head, neck, TMJ and oral cavity. It is only with a combination of both the X-ray and the visual examination that the dentist can best treat his or her patients. Some people worry about their exposure to radiation during dental X-ray procedures. This is very understandable in light of the relatively high radiation of some medical X-rays. Dentists are very concerned about minimizing the amount of radiation a patient receives at the dental office. That’s why Denmark Dentistry has invested in Digital X-Ray technology and intra-oral cameras. Digital X-rays offer the advantage of an 80 percent or greater reduction in radiation, no need for film or processing chemicals, production of a nearly instantaneously image, and the ability to use color contrast in the image.

One of our Dentists will go over his findings related to your current dental care needs and concerns. If additional treatment is needed to addresses your dental care needs or concerns; one of our Dentists will discuss treatment options with you, so that you can make the best choice for your dental care. Don’t hesitate to ask questions, it is very important that you understand these conditions and what they mean for your dental and overall health. Once a course of treatment is agreed upon, a plan and timetable will be constructed for you. Financial options will also be discussed.

Denmark Dentistry accepts most major insurances and is a preferred provider for many plans. During your registration process, our front staff will verify your insurance coverage so that we may provide you with the best estimate possible of your treatment costs.
Payment for professional services is expected when services are rendered. We accept Visa, Mastercard, Discover, checks, and cash. No interest payment plans through CareCredit may be set up in office. Our front staff will be happy to answer any questions you may have about payment options.

Driving Directions
Denmark Dentistry is conveniently located in front of Denmark-Olar High School in Denmark, SC.

From Bamberg: Take HWY 78 (Heritage Hwy) to Denmark. We are on the right hand side once you get to Denmark in front of Denmark-Olar HIgh School.

From Blackville: Take HWY 78 (Heritage Hwy) to Denmark. Keep going like you are heading to Bamberg.  We will be on the left hand side of the road in front of Denmark-Olar High School.

From Orangeburg: Take Hwy 301 towards Bamberg. Turn right on HWY 70. Turn left onto HWY 321 in Denmark. Turn left onto HWY 78 at traffic light. Keep going on HWY 78 (Heritage Hwy) like you are heading to Bamberg.  We will be on the left hand side of the road in front of Denmark-Olar High School.

From Olar: Take HWY 321 to Denmark. Turn right on HWY 78 (Heritage Hwy) and come down like you are heading to Bamberg.  We will be on the left hand side of the road in front of Denmark-Olar High School.
